  • 2008-2009 Dependent Verification Worksheet (PDF) - This form is only for dependent students who were selected by the US Department of Education for review in a process called "Verification" for the 2008-2009 financial aid year. This form is used to compare the financial information on the Student Aid Report and financial documents. If you were not selected for this process by the US Department of Education, you need not fill out this form.
