I-20 Reactivation Request Form

Remember: The travel signature is valid for 12 months from the date it was signed last

In order to process your re-activation I-20 request, you must provide a copy of your return flight ticket or travel itinerary. Please keep in mind that you will not be able to enter the U.S. or sign up for classes until your I-20 is re-activated.
